Health insurance
How Much Health Insurance Do You Need in India?
One serious hospital stay at a top private hospital in Kolkata, Delhi or Mumbai is billed about ₹5.5 lakh today and about ₹1.3 crore by the time a 35-year-old turns 80. How we got there, in four steps and eight charts: CGHS rates, private hospital tariffs, city prices, our own claims and medical inflation, with every source listed.

Single private AC room, no existing illness. Every number below comes from a source listed at the end.
The answer in four steps
- Price a serious event with a government rate: CGHS, ₹1.80 lakh for heart bypass surgery.
- Re-price it privately: 2.5× for the best hospitals, 1.75× for a good one, plus five ICU days.
- Adjust for your city: from 1.00 in Delhi, Mumbai and Kolkata to 0.65 in smaller cities.
- Make it last until you are 80: medical inflation of 10% a year for 10 years, then 8%, then 6%.
Figure 1
From a government price to ₹5.5 lakh
- CGHS rateheart bypass, NABH, Tier I city₹1.80 lakh
- × 2.5 best private hospitalprivate room₹4.50 lakh
- + 5 ICU days₹20,000 a day₹5.5 lakh
City factor 1.00 (Kolkata, Delhi NCR, Mumbai). For a good private hospital the same steps give ₹3.9 lakh.
1. Private hospitals charge 1.75–2.5× the government rate
- CGHS publishes fixed package rates. Private hospitals publish their own schedules, and we compared the two procedure by procedure.
- For surgery, private prices run 1.3–2.5× CGHS, and up to 3.9× at the top end.
- Per-day charges carry the biggest markups: ICU costs 2.0–4.2× CGHS.
| Procedure | CGHS rate | Private, published | Private ÷ CGHS |
|---|---|---|---|
| Heart bypass (CABG) | ₹1.80 lakh | ₹2.5–7.0 lakh | 1.75–3.9× |
| Angioplasty + 1 stent | ₹1.37 lakh | ₹1.5–2.8 lakh | 1.3–2.0× |
| Knee replacement, one knee | ₹1.52 lakh | ₹1.5–3.5 lakh | 1.3–2.3× |

One day in ICU: government rate vs private hospitals
Hospitals' official schedules of charges, 2019–2025. ICU charge per day; the CGHS rate includes the room.
2. Your city changes the bill by up to 35%
CGHS city tiers do not match private prices. Gurgaon is CGHS Tier II but prices like Delhi. So we grouped 20 cities by what their hospitals actually charge for rooms, ICU and heart surgery.
Figure 3
City price factors (Delhi = 1.00)
Lucknow, Bhopal, Kochi and Surat have no published tariffs; they take the group default.
3. What our own claims show
Our claims are a check on the estimate, not an input to it. We analysed 161 claims with a full hospital bill from three employer group health policies we service (2024–26, mostly Kolkata and Bengaluru). Only totals were used; no individual's data.
Figure 4
Most bills are small. The big ones decide whether your cover is enough
Share of the 161 hospital bills. Policies with ₹1–2.5 lakh cover ran out on 13% of claims; one group with ₹2–4 lakh cover was paid only 38% of what was billed.
Figure 5
For every ₹100 billed, insurers paid ₹73. More cover would not have closed most of that gap
- Most of the ₹27 was room-rent deductions, non-medical items and “reasonable and customary” cuts. A deduction never uses up the sum insured, so a bigger policy pays none of it back.
- The fix is plan features: no room-rent cap or the right room category, and consumables cover. The calculator lists these add-ons for each plan.
- So we size cover to the hospital bill itself, with no extra margin for deductions. ₹5.5 lakh is above 97.5% of the bills in our sample.
Figure 6
How often people are hospitalised in a year
Same pattern as the national survey: claims rise sharply with age. Parents made about half of all paid claims in our groups, which is why the calculator sizes them on a separate policy.
4. Make it last until you are 80
Raising cover later means fresh waiting periods on the extra amount and new medical checks, which may be refused once you are no longer fit. So the cover you buy while healthy has to keep up on its own, until the person buying it turns 80. For parents, we plan until the eldest parent turns 80, and always for at least 10 years.
Figure 7
Medical inflation in India: four estimates
Private-sector estimates describe employer health plans and include higher usage. The national survey measures price only. We start at 10%, the middle of the range, then step down to 8% for years 11–20 and 6% after that, as no one expects today’s pace to last 45 years.
Figure 8
For a 35-year-old: ₹5.5 lakh today, ₹1.3 crore at 80. A fixed ₹10 lakh policy falls behind in year 7
The Figure 1 hospital stay, priced every year to age 80. Each plan grows its cover differently (bonus, booster, supercharge), so the calculator finds the smallest starting cover in each plan that stays above this line every year.
5. Family, age and health
| Factor | Adds | Why |
|---|---|---|
| Each extra adult | +25% | A second big claim in the same year |
| Each child | +10% | Children's bills were the smallest in our claims |
| Family total | capped at 1.75× | Several big claims in one year are rare |
| Anyone 60+ / 75+ | +15% / +30% | Hospitalisation nearly triples after 60 (NSS) |
| Existing illness: mild / serious | +10% / +25% | Our assumption; no public data |
| Parents | own policy | About half of all paid claims in our groups |
What it adds up to
| Family | Need today | Need at 80 |
|---|---|---|
| Single, 35, Kolkata, best hospitals | ₹5.5 lakh | ₹1.3 crore |
| Couple 32 and 30, Bengaluru, good private hospital | ₹4.1 lakh | ₹1.2 crore |
| Couple 38 and 35 + 2 children, Kolkata, best hospitals | ₹8 lakh | ₹1.6 crore |
| Parents 65 and 62, Kolkata, good private hospital | ₹5.6 lakh | ₹21 lakh |
“At 80” means when the proposer turns 80; for the parents, when the elder turns 80. The calculator turns this need into a starting sum insured for ICICI Lombard Elevate, TATA AIG Medicare Premier and TATA AIG Medicare Select, side by side and unranked.
Where this method is weak
- Our claims are a small sample: 161 bills, three employer groups, mostly eastern and southern India.
- Hospital tariffs are 2019–2025, and some cities have no published prices.
- The illness loadings are our judgement, not published data.
- Inflation over 45 years is a guess. 10%, then 8%, then 6% is our assumption; today’s published range is 8–13%.
- Transplants and advanced cancer care (₹15–60 lakh today) can exceed any of these numbers. Plans with a one-time unlimited claim cover that tail.

Questions this raises
How much health insurance do I need in India?
Enough to pay for one serious hospital stay at the hospital you would use, today and every year until you are 80. At a top private hospital in Kolkata, Delhi or Mumbai that stay is billed about ₹5.5 lakh today and about ₹1.3 crore by the time a 35-year-old turns 80, assuming medical inflation of 10% a year for 10 years, then 8%, then 6%. A couple with two children needs about ₹8 lakh today.
Is ₹5 lakh health insurance enough?
Only for today, and only at a good rather than a top hospital. A major surgery with five ICU days at a good private hospital in a metro is billed about ₹3.9 lakh today, but about ₹94 lakh by the time a 35-year-old turns 80. A fixed ₹5 lakh policy falls behind within a few years unless its cover grows every year or a super top-up sits above it.
How much more do private hospitals charge than CGHS rates?
Published private prices for surgery run about 1.3 to 2.5 times the CGHS package rate, and up to 3.9 times at the top end. ICU charges per day run 2 to 4.2 times the CGHS rate.
What medical inflation rate should I assume in India?
Current estimates range from 8.3% a year (NSS hospital costs, 2017 to 2025) to 13% (Milliman's 2025 projection). Aon puts 2026 at 11.5% and Mercer Marsh Benefits at 9.9%. For a plan that has to last to age 80 we use 10% a year for the first 10 years, 8% for the next 10 and 6% after that.
Should I buy extra cover because insurers do not pay the full bill?
No. In 161 claims we analysed, insurers paid ₹73 for every ₹100 billed, but most of the gap was room-rent deductions, non-medical items and reasonable-and-customary cuts. Deductions do not use up the sum insured, so more cover would not pay them. Choose plan features instead: no room-rent cap or the right room category, and consumables cover.
